Case Manager - Meth Diversion

Overview

This position will function as a social worker within the Behavioral Health Unit, specifically providing case management services for the Meth Diversion Program and crisis follow up as statutorily required under Chapter 34. Duties further include work with Chapter 51 involving individuals with chronic mental health concerns and providing them with ongoing services. The position would provide the ongoing, court ordered case management for Meth Diversion participants (as funded through the TAD grant) as well as next day follow up to those individuals seeking services through the 24 mental health crisis line. Meth Diversion participants are provided intensive case management and oversight as they are kept in a community based setting while seeking AODA services. Crisis follow up services are not only statutorily required, they can also provide community based services and referral information with goals of keeping individuals in the community versus more restrictive settings. Additionally, this position may provide Comprehensive Community Service (CCS) Facilitation services as per Wisconsin Administrative Code DHS 36 as part of the ongoing caseload. 

 This position will include being part of the evening mobile crisis response rotation that will require after hours intake one week, every 5-7 weeks, from 4PM-12PM.

  • Wage: $23.79 - $25.07 per hour DOQ

Requirements

Qualified candidates will have a Bachelor’s Degree in Social Work, Psychology, Guidance and Counseling, Criminal Justice or closely related field. Current Social Worker Certification preferred. An individual holding a current Wisconsin Social Work Training Certificate can be considered. Two years of post-graduation professional case work at the type and level consistent with a County Human Services Worker. Ability to pass required background checks, and a valid driver's license required.
