The goal of the Long Term Support Unit is to help older adults and people with disabilities remain safely in the community by providing advocacy, support, protection and coordination of services.
Funds from Federal, State, and County Programs are available to assist eligible residents in need.
Community Options and Medicaid Waiver Programs
These programs enable clients with long term support needs to remain in the community by providing assistance in the following areas:
Birth to Three and Family Support Programs
These programs recognize the emotional, physical and financial hardships placed on a family with disabled children. They provide individual services and supports to meet the needs of the family.
Adult Protective Services
Provides services for vulnerable adults who might be physically / financially abused or who need help in making life decisions.
ADULT FAMILY HOMES AND GUARDIANS NEEDED!
Our agency certifies one to two bed adult family homes. Homes are needed to provide safety and supervision for adults no longer able to live alone, but not yet ready for a nursing home.
We will assist individuals and families through the court process to establish a guardianship. We are in need of people willing to be guardians for those without family and friends available.
