Burnett County Community Health Assessment

What is a community health status assessment?

A community health assessment is an evaluation of the health of our community. It answers the question: How healthy are our residents and the environment in which they live and work? The assessment is important because it helps us to understand our community’s overall health, specific health status issues, and priority needs.

This is a short survey that will take about 8-10 minutes to complete. Your participation in the survey is voluntary, completely confidential, and anonymous. Your computer’s IP address will be deleted from the database thereby eliminating personal identification.

Those who live (part or full-time) and/or work in Burnett County, are eligible to take the survey.

Participating in the community health assessment survey will benefit you. It will give you the opportunity to voice your concerns about the health of our community and highlight the areas that you identify as high priority. This, in turn, will impact future planning regarding programs and services offered by the Burnett County Department of Health and Human Services.
